You will have already, or will shortly receive a leaflet from the NHS entitled “Better information means better care”.
If you would like to tell us about your preference regarding the recent leaflet sent to you from the NHS entitled “Better information means better care”, we will only be able to record your preference if you include in your message the following information:
- Your name in full
- Your date of birth
- Your address
- We will need to know if you would like to opt-out of sharing information help by GP practice
- We will need to know if you would like to opt-out of sharing information held by other care providers e.g. hospitals and community services.
